The cardboard thin Seesaw Mobile Phone designed by Karsten Willmann offers soft keys and a small Seesaw-like lever in the center-back, which hides the phone's camera and flashlight.
At this point, the phone is merely a concept, but its tiny size and the fact that it folds into an even smaller flip phone makes me think it would be a tremendous hit with users.

4This is probably one of the smallest ones that I have seen in the U.S., but from my observations, other countries tend to have phones that are a lot smaller.
