All of the above! Although, I don't use a pillow, I use one of those laptop lap desks things you can get from Best Buy. I work best if I stick to my home office, though.
Berichard, the old Mac laptops had the logo upside down. They finally smartened up and put it so it faced up when open.
Anyway, I use my MacBook everywhere, including, I'm afraid on my lap or on the bed. However, I finally smartened up and use it on a hard place when it began to overheat.
I pretty much take my laptop everywhere. I will occasionally take it into the bathroom so I can keep chatting or watch something online while I paint my toenails.
I've got big curved lap desk, wide enough that you have the option of resting the sides of it on the arms of chairs in addition to putting it in your lap. I've also got a notebook cooler
thing I put underneath the laptop most of the time. I have been amazed how much cooler that keeps the laptop.

2I typically lay on my bed with my laptop on my lap, which probably contributes to my poor posture!

10I use it on my lap, but I try to keep the fan area open. I tried a lap tray, but it was just cumbersome and uncomfortable.

12I'm usually on my bed (like I am right now), with the Ikea thing under so it won't get too hot. Sometimes in the kitchen or coffee table.
